Terms and Conditions

1. Algemeen

  1. During the initial interview, the therapist will explain to the client about the acupuncture treatment and expected effects;
  2. Before or during the first treatment, the client will provide information about:
    - the main complaint;
    - health and medical history;
    - be treated by a doctor, specialist or therapist and for which complaint / condition;
    - the medicines used;
  3. During the treatment period, the client will inform the therapist about any changes that occur in relation to the information provided previously;
  4. When paying for treatments, we assume that the customer agrees with the terms and conditions and the privacy policy. both can be accessed through the Dami Acupunctuur & Welzijn;
  5. If the client is being treated by a doctor, specialist or therapist, the client will ask if acupuncture fits into the treatment plan;
  6. Acupuncture is not a substitute for regular medical care. If the customer is being treated by a doctor, specialist or therapist, he/she too will remain under the supervision of that physician during the treatment period and the client will comply with prescribed medication and guidelines.

2. Treatments

  1. The customer requests an inquiry by phone, email or via the website. This will be confirmed by phone by the therapist within 48 working hours.
  2. During the first consultation, the client's complaints and wishes are mapped out into a treatment plan by the therapist, which may consist of several treatment sessions;
  3. The customer agrees to the current rates for a treatment session. Fees may vary by treatment and duration of treatment. This is always discussed with the client prior to treatment;
  4. Payment must be made prior to treatment. The therapist confirms the payment terms with the client over the phone. The customer will then receive a tikkie to make the payment.
  5. The cancellation of an appointment is possible free of charge up to 12 hours before the scheduled time. In case of cancellation less than 12 hours in advance or failure to attend the appointment, the payment for the session will not be refunded.

3. Responsibility

  1. The customer agrees to be treated at his own expense and responsibility;
  2. The therapist is not responsible for damages and / or injuries resulting from the retention of medical information by the client, such as physical and / or mental disorders, injuries and use of medications;
  3. The therapist does not provide medical assurance and is not responsible for (medical) complications that occur during or after treatment with the client, which are not due to serious disabilities attributable to the therapist. The therapist is in no way responsible for medical damage.

4. Complaints

  1. If the client has any complaints about the therapist and/or the treatments, they should discuss this with the therapist.
  2. If the therapist and the client cannot reach an agreement, the client can explain the situation in writing and send it to info@damiacupunctuur.nl

5. Data processing

  1. The practice complies with the AVG Act. The way in which customer data and privacy are treated is stated in the Privacy Policy which can be found on the website.

These general terms and conditions are part of the treatment agreement. By signing the treatment agreement, the client agrees to these terms and conditions.
