      1. For good treatment it is necessary that I, as your responsible therapist, create a file. This is a legal obligation imposed by the WGBO (Medical Treatment Agreement Act). Your file contains notes about your health and information about tests and treatments performed.

      2. The file also includes information that is necessary for your treatment or that I, after your explicit permission, have requested from another health care provider or that you have provided yourself.

      3. We guarantee your privacy by:<\/span>
        <\/span>- \u200b\u200bTreating your personal and medical data with care;<\/span>
        <\/span>- As your therapist responsible for treatment, only I have access to your file data. I have a legal duty of secrecy (professional secrecy).\u00a0

      4. Your file data can also be used for the following purposes:<\/span>
        <\/span>- To inform other healthcare professionals, for example, when therapy is completed or in case of referral to another doctor. This only happens with your explicit permission.<\/span>
        <\/span>- To recommend Chinese herbs and\/or supplements at Natural Pharmacy.<\/span>
        <\/span>- Name, address, place of residence and date of birth are used for financial administration so that I or my administration can issue an invoice. If you give permission for this in the treatment agreement (you sign it before the first treatment).<\/span>
        <\/span>- For anonymous use during peer review.<\/span>
        <\/span>- For anonymous use for training purposes (during internships and\/or in my (digital) classes).

      5. If I want to use your data for any other reason, I will first inform you and explicitly ask for your consent.

      6. This data on the client's file is kept for 20 years as required by the Medical Treatment Contract Act (WGBO). Data is automatically deleted 20 years after the last treatment. You have the right to view this data, to have it adjusted or, upon written request, to remove it in advance.
